Effective December 2, 2019, there are new instructions, forms and events for filing an Application for Payment of Unclaimed Funds.  

The Instructions are available on our home page and on the Unclaimed Funds Forms page.

The forms include U-1a Notice of Motion, U-1b Form 1340, Application for Payment of Unclaimed Funds, and Proposed Order U-2(a) Granting Application for Payment of Unclaimed Funds.

The events in CM/ECF are Notice of Motion and Application for Payment of Unclaimed Funds and Unclaimed Funds Supporting Documentation. All supporting documentation, as outlined in the Instructions, should be filed with the Unclaimed Funds Supporting Documentation event, which is restricted from public access. 

Continue to schedule the motion before Acting Chief Judge A. Benjamin Goldgar on Mondays, in Courtroom 642, on a date when the Judge is sitting.
