The SSN Statement should be signed by the debtor, scanned and filed using the event, Statement of Social Security Number(s). Please be sure your scanning equipment is set to portrait and the document is right side up.

If the signature is inadvertently omitted from the document, please refile the document with the actual signature of the debtor(s), not a /s/. Please be sure to include AMENDED on the document and file using the event, Amended Statement of Social Security Number(s). For procedure on how to file, you may access the Amended Statement of Social Security Number(s) procedure, which is available in the on-line manual.

Do NOT include the SSN Statement with the petition package.

Do NOT include an unredacted SSN Statement with other signature pages.

Effective December 1, 2014, if an unredacted Social Security Statement is included with the petition package or any other filing, you will be required to file a Motion to Redact and pay a $25.00 fee.
