Form B2830, Chapter 13 Debtor’s Certifications Regarding Domestic Support Obligations and Section 522(q)

Effective immediately, please use Form B2830, Chapter 13 Debtor’s Certifications Regarding Domestic Support Obligations and Section 522(q) to certify that the requirements of 1328(a) are met in order for the Court to issue the discharge. Remember, this form must be filed AFTER the trustee files the Notice of Completion of Plan Payments. We have made the form fillable. It is listed under our Local Forms, in the Chapter 13 section, as form 13-15.
