Effective Monday, April 16,2007, Official Form B-21, Statement of Social Security Number, must be submitted with every petition for an individual debtor. The form is required per Bankruptcy Rule 1007(f). Currently, the Statement of Social Security Number has not been a required document for electronically filed petitions. This change is being made to insure that the Court records are correct and complete. The Statement must be entered as a separate filing, using the event, Statement of Social Security Number.
