Revised Declaration Regarding Electronic Filing

Effective July 1, 2005
The Declaration Regarding Electronic Filing, required when filing a petition electronically (ECF) or on diskette, has been revised. For installment cases, the signature of the debtor(s) on the “Declaration” will take the place of the signature of the debtor(s) on the Application to Pay Filing Fees in Installment. The installment Application can be completed using the /s/ for the signature of the debtor(s) and filed in PDF format.
There are two versions of the Declaration Regarding Electronic Filing form. One form to use when filing a petition electronically (ECF) and another when filing a petition on diskette. The “Declaration” to use when filing a petition electronically (ECF) only requires the signature of the debtor(s). The “Declaration” to use when filing a petition on a diskette requires the signature of both the debtor and attorney.
