Additional Information on Amended Administrative Procedures for CM/ECF Effective 11/01/14

Important Change to Documentation for Original Non-Attorney Signatures, Section II.C.1.

This change affects signatures of debtors or other non-registrants (non CM/ECF filers) and requires that the signature page of a document be attached to and submitted with the document in a single filing.

The signature page should contain the actual signature of the debtor or other non-registrant, not a /s/.

For the petition package that has multiple signature pages, the pages can either be inserted in the package with each document or all signature pages can be added at the back of the document. Here is an example of a bookmarked petition with the signature pages at the back.

As an alternative, the signature pages can be scanned and added as an attachment to the petition entry.

"Chapter 7 Voluntary Petition Fee Amount $0, Filed by Attorney on behalf of Debtor. (Attachments: # 1 Signature Pages) (Entered: 10/29/2014)"

If the signature pages are inadvertently omitted from a multipage document, please file them using the "Attachment" event and relate to the appropriate document.  

For one page documents, such as Debtor's Certification of Completion of Instructional Course Concerning Financial Management, the form should be signed by the debtor and scanned. Please be sure your scanning equipment is set to portrait and  the document is right side up.

The local forms, Declaration Regarding Electronic Filing of Petition and Accompanying Documents and the Declaration Regarding Electronic Filing for Documents Filed After Petition will no longer be required.

This change is effective November 1, 2014.
